Case Management / Rehabilitation
Hope Horizon Mental health has a board certified child psychatrist, licensed clinical staff, registered intern clinicians, student interns and trainees, and mental health rehabilitation professionals.
Case Management
This service helps with getting medical, educational, social, rehabilitation, or other community services that may be difficult for people with mental illness to access on their own. Targeted case management includes plan development, communication, coordination, referral, and monitoring service delivery.
